OOTB Record Picker with "create new" option

This stuff should be OOTB. I used a modified version of Stefan's custom picker https://appian.rocks/2022/12/22/create-new-items-using-a-custom-picker/ but ran into a couple of rather maddening edge cases (that had nothing to do with Stefan's example) that took me a while to figure out.
