Modeler should show Model / Instance name in the huge empty title bar

The modeler wastes a lot of space in the top bar which only contains the "appian" logo otherwise; it should show the name of the current model or instance open in the current modeler tab, especially since when all available tabs are open, it's impossible to otherwise see the name in question without mousing over the selected modeler tab.
